A 45 year old gentleman, Mr X, was referred to our dietetic-led gastroenterology clinic in 2016 with symptoms of severe bloating, wind, abdominal pain, urgent diarrhoea, gurgling and very low energy levels.

Mrs X was a married 35 year old with two young children. She was self-employed and her job entailed extensive travel. Prior to her referral into our coeliac and gluten sensitivity service she had presented to her GP complaining of fatigue and gastrointestinal symptoms.

Mrs X, aged 42, was referred in October 2014 to the general dietetic community clinic in Somerset for dietetic intervention for her symptoms of IBS.  

She had suffered with diarrhoea dominant IBS since her teens.  However, it is vital to look back to infancy/childhood to get a full case history and hints on what might be the most effective dietary treatment.

Sophie initially presented to the paediatric emergency department aged 2 years 3 months with extreme lethargy, poor oral intake and weight loss.  Parents reported she had not been well for 5 weeks, initially with diarrhoea for 2 weeks, vomiting for a week then constipation and poor oral intake. She had previously been fit and well. Due to a normal examination, she was diagnosed with temporary lethargy post viral gastroenteritis (shigella was found on a stool sample), and not admitted but given an outpatient follow-up for two weeks.
